Extracurricular Activities (Clubs, Sports Teams, Organizations)

List ALL activities and year(s) that the student participated in while in high school. If the student held a position (e.g., Head Girl, President, Secretary, Captain, etc. please specify that near the activity).

Acknowledgement

I acknowledge that each high school transcript request will incur a $10 charge. If the student did not attend grade 10 and/or 11 at ALI, you must send the completed report card from their previous school for those years. No transcripts will be produced if a balance is owed on the student's invoice. Final transcripts will only be emailed directly to the institution or can be picked up in a sealed envelope.
